There was a bug in prior versions where leagues without waivers had no waivers/DFA screen, so players "disappeared" in the manner you guys are describing.
I believe this has been addressed by now allowing you to access the DFA area through the drop-downs in the Transactions screen, even if your league does not have the Waiver rule enabled.
I'll let you know if I figure out more.
