AFAIK, if you check the box next to "Major American League" you'll get MLB, the Pacific Coast League, the International League, etc. all as they should be with the correct cities (although the nicknames will be made up). It's only if you create a new MLB from scratch and then use the "Add all levels of minor leagues" option that only one affiliate per team for each level is added.
