I just had the same thing happen to me using the latest Patch 3 (2.0.3.68). Was playing out a game - my pitcher got hurt - and as soon as the PbP was over, game crashed to desktop with the same message. I did not notice if it was the last out of the inning or not. So this is still not fixed - hopefully it can be found and quashed before OOTP 9.

I think it being the last out of the inning is the key factor here. Somehow OOTP isn't processing this part properly.

I hope this doesn't happen on every pitcher injury, because I like to play out all of my games. If the game crashes every time a pitcher gets hurt, then none of my pitchers are ever going to get hurt, which is extremely unrealistic. If that's the case, this issue really should have been focused on for inclusion in the last patch. If it's only on the final out of the inning, I'll just tell my pitchers to be real careful getting that third out. :
