Excuse my ignorance on these matters, but after the batter is out of the box is it really still the manager's word that makes the runners choice to go up more than one base?

I agree out of the box for sure though - I've been burned by a double trying to be stretched into a triple in a late game situation when I would have liked to have told the hitter to stop at second.
