Unless things have changed in the last 20 years or so,
officially that player would go from pinch-hitter (or pinch-runner) to "substitute," although I've certainly never seen anything in a boxscore like "PH-Sub."
This being the case, a pinch-runner batting later in the inning is not batting as a pinch-hitter; and a pinch-hitter batting for the second time in an inning is not batting as a pinch-hitter the second time.
Probably this is pretty obscure because as far as I know, there aren't any records kept for "substitutes."
