Yes

When you write that "the rotation says to start the #5 pitcher", what do you mean exactly? Will the AI actually have the #5 starter pitch, or does the Pitching tab simply have the fifth starter marked as starting next?

Both. The pitching tab show the fifth starter and this is indeed the starter when you go to play the game.

I have since, played on and it seems that the fatigue is not right. I don't think a pitcher should be fully rested two days after throwing in excess of 100 pitches.

Also, in continuing, the AI is acting as though "strict order" has been chosen for the rotation mode. Has never skipped the next sequential starter even with off days and higher ranked starter being fully rested.

Perhaps it just says the starter is fully rested when in fact he is not??
I am wondering about this because in playing the game, I have had a starter still be OK (at least that is what it says) after throwing 120 pitches.
