One of my main dislikes: Commissioners who won't make mid-sim changes for a team. For example, activating a player off of the DL, or following any other management instructions and requests that an owner might make. And this is just one aspect of something that, in my opinion, makes a league dysfunctional: that's when a Commissioner is too "hands off" and doesn't directly pilot his league along its course. In the worst case, a Commissioner can start a league, and then just let it run, without keeping close control over its organizations and outcomes.
A Commissioner needs to know what every owner is doing, at all times. He needs to know what's going on with every player on every team. He needs to know what's happening in every at-bat of every game. A league will go to seed if there aren't multiple sets of eyes watching its every corner. For a league to stay healthy in the long run, Commissioners must be ever-vigilant. Commishes must monitor statistical output every season, making sure that the league totals conform to the league's goals and guidelines. A Commish must watch over the personnel of every club, catching and correcting owners' mistakes. The Commissioner must be constantly modifying and improving his league's details, whether it's customizing the website, making new uniforms, or modding the league forum.
I've used the analogy many times: a league is a garden, which the Commissioner must constantly tend.

I've just recently realized how important that an inaugural draft is. Having an inaugural draft, or not, means the difference between having league parity, or not having it, for the first several seasons of a league's history.

I like these things too, but not all leagues have a lot of visible activity, and general public activity just isn't a part of some leagues' personality. Many leagues leave extraneous storylines and embellishments to the owners' individual imaginations.

Freedom in financial matters is good, but leagues must work to prevent abuse of the game's financial system. Leagues need to consider whether contact incentives should be allowed. If a league uses financials, the league Commissioner needs to run tests, and model the financial environment of upcoming seasons. The Commish also needs to watch all teams' financial activities and situations carefully. Everything from ticket prices to player contracts must be under constant scrutiny.

I have the opposite opinion on this one. I have no use for real-player leagues, just as I have no use for real-world Major League Baseball.
