Upon reviewing my templates folder in my custom install location, the team_report_leaderbox_frame.tpl file was indeed missing. So I just installed the full version of patch #4 into a folder on my desktop and then copied all the files from the template directory into the template directory in my custom install location and that resolved the issue. Your post led me in the right direction, mtopham. Thanks.

It appears there is a problem with running the upgrade as opposed to the full install.
