A fighters rank is determined by his last 10 fights and the amount of performance points received in those fights. A fighter could realistically lose his first 30 fights, win his next 10 and be ranked #1. That's how the game itself works. As for regional titles, they are often given title shots for their performances with fighters just from that region. Let's say Joe Boxer is trying to contend for the Montana Boxing Association Heavyweight title. He fights 3 times a month, but only 1 time every two months in Montana. He loses all his other fights but the Montana one. The people of Montana think this guy is king, though, because they never see him lose, thus the title shot due to his "local" record.
