What exactly seems to be the problem? I don't have OOTP6.5 installed right now and I'd rather not re-install it, but maybe I can help you from memory. Besides, I don't think the options on that screen have changed much so we can probably reference OOTP9's screen.
EDIT: With reading that page that I linked above you might not even need anyone's help.

Hmm.. Well, are you sure you have those folders created on your ftp site? If you don't know how, you can check it by accessing your ftp site through either an ftp client like filezilla or a web-based app your host provides.
If you do have those folders created (something tells me OOTP6 only allowed for one folder, but I'm not sure), then are you making sure that you're prefixing their names with a forward slash (/) as in the guide above? If that's the address that you access your website from on the web then it's most likely the same as your ftp address. You could try prefixing it with "ftp." though as in the guide above. If you're pretty sure the folders are there and entered correctly and the ftp address is correct as far as you know then you may need to contact your webhost. Fidel's tutorial appears to be written for OOTP6 so maybe it might be of some help too.
