Contacting e-License for license reset?

I've re-installed OOTP 2006 on my 'puter after upgrading and rebuilding it, but I'm unable to use my license. I read that I should contact e-License to get it reset, but who and where should I contact, and what should I tell them?
I don't want to make a mistake and risk my license being permanently invalidated. Thank you for your help.

http://www.elicense.com. Use the support link and explain the situation. They will ask a couple of questions to verify your information and will reset everything. There is not any risk of your license being invalidated by using their support. Please post back and let me know if everything works out.

Should be enough to get a license reset from eLicense.
