Disappearing Players from Minor Leagues

I run a online league and the past two seasons we been having problems with
Minor League players disappearing.
If you look at the teams Transaction screen the Minor league teams either show empty or a few players but when you go to the Minor League System Report it shows all the players.
I end up having to find all the missing players and put them back on the respected teams. So if the player says he's on the AAA team I have to place them on the AA team for them to show back up

It's not ever team ever season but I'm wondering if I'm doing something wrong?

Still having the same problem after I fixed everything the next time I sim it all goes back to the way it was (some teams with no players in the minor leagues)
