Post can't sub pitcher

Well, this time I wasn't thrown out of the game, but nonetheless, when my starter got tired after a rain delay, I warmed a guy up but couldn't put him in. Everything else was fine. Then I edited the starter to make him no longer tired, but he still appeared red. I couldn't tell if he suffered from fatigue, b/c he's a very good pitcher.

How did you try and enter the reliever, which of the two pitchers slots did you try and place him in (the top one or the one in the lineup)? Is there a chance you tried to put the pitcher in wrong spot? (Obviously if you have a DH there is only one slot)

I had a DH, so I was taking him out of the first bullpen slot and placing him right on top of the current pitcher - the same thing I've always done. I had all the settings to me managing when I started - is there some way I could have accidentally changed that mid-game (though that would be unlikely)?

Don't know if this is related:
Playing as the Dodgers, in an interleague game. Came to substitute my starter, and there wasn't actually a visible 'currently pitching' line. However I managed to do the substitution by dragging the new pitcher to just above where it said 'Bullpen'

Later in the game I made some more substitutions and got in a position where I'd have really liked to put my DH in as a replacement 1B, but the game wouldn't let me, pointing out that the pitcher would have to bat. Hey, I'm managing a NL team, the pitchers are used to batting , but I can see how it could cause problems for the game

If it is an interleague game and the DH team is home you play by their rules. Plus in the othe situation about subbing for the pitcher, in a DH game that is where you have to sub him to...just above the bullpen.

I have no problem with having to keep the DH in, just thought it slightly amusing that the reason given in the message from the game was that the pitcher would have had to hit, which isn't really a problem for NL teams.

As for the subbing, I recall V3 showing the current pitcher above the bullpen, but he wasn't shown when I was playing yesterday. I had to hit a spot that was basically just an extra thick line

It happened again. I haven't tested it, but I think I know what it is. I suspect that it happens if you put a pitcher in while you're at bat (i.e., b/c you know you're changing pitchers and there's no reason to keep him warming up). After that, it seems, you can't sub for that pitcher. Maybe it has to do with the rule about facing one batter - the program never reads the sub pitcher as facing a batter for those purposes?
