I have a pitcher in my starting rotation and no matter where I put him (starting in the majors, bullpen in the majors, starting in the minors, etc.) the AI will not use him. If it is his turn in the rotation and everyone else is tired, the AI will start one of my middle relievers. The only game he's pitched this year was a game I played out where I had to insert him into the rotation. It was his spot, but the lineup screen came up with no pitcher selected prior to the game.

I'm using the latest patch and have the save game.

Edit: Actually, I made a backup and then traded the guy and now the computer team has him in their rotation but he never pitches for them. I've simmed forward a couple weeks and it lists him as the next starter every game, but then the AI chooses either a SP that isn't exhausted or uses a reliever.

I've had the same issue with a couple of starting pitchers. I trade them out of my organization but AI still refuses to use them.

That is odd. Even works like that when you set your rotation to "strict order" ? Would he happen to have less than 3 pitches he can throw?
