No. Whether that's a good reason to stop playing OOTP seems a little extreme. Overall, the AI's pinch hitting decision making got dramatically better in OOTP. There are still some questionable things, but it's not all that far off from what happens in real life.

You can if you play the game in PbP, but I'm assuming the OP is talking about setting perfered players at PH. That is a pretty interesting idea, but I think the AI does a pretty decent job of picking a PH when needed. I mean, it only has a couple choices...

Each game has unique situations. There could be injuries, defensive subs etc. Hard coding the AI to use a specific PH could cause other abnormalities that you may find inconsistent with reality. The AI does a decent job as is.

The AI will sometimes get fixated on using a particular player as it's first choice to PH, so this can be annoying if you think the player is a poor choice to PH. There is nothing you can do with settings or the like to fix this. If it's annoying enough, the fix is to take that player off your active roster (or trade him, or whatever). Not the most elegant solution, but practical and doesn't usually diminish the game all that much. It's not a good reason to walk away from OOTP.

There is a logic problem with your question.
If you are managing the game, then you have complete control over who pinch hits. This works in the game now. If you are the GM you may be able to set the lineup (not normal IRL) but you must depend on the field manager to substitute handle pitchers etc. If you're willing to do that why do PH make such a difference. Handling the pitching staff is way more of an issue to me than PH. If it can be added, that'd be fine.
