Pitching change/new inning ...

If I bring a new pitcher into the game to a fresh inning (like make the change when my team is batting - so he runs out to the mound with the defense) - is the new pitcher required to have been working in the bullpen (for optimal effectiveness) ?

Not if you're playing with Warmup off. If you're playing with Warmup on, the game may treat the pitcher as if he were cold.
