BZ 3406: Injury replacement in pbp

When a fielder is injured on a play that results in the 3rd out, there should be no need for the team to place a new player in the lineup until either a) that position in the lineup comes to bat, or b) the team returns to the field

I logged this as BZ #3406 with comment as follows:

In version 9, I believe, this was addressed and either marked LATER, WONTFIX, was treated as a feature request, or there was a response that it could not be fixed, but the point is valid and needs reconsideration.

This if further complicated when the player you are forced to substitute fails to ever actually take the field, e.g. being PH for himself before the next AB, or in some rare cases - when a MR is inserted in pitching slot - the MR may be credited with an appearance even though he did not make one (again, if a PH is utilized prior to his AB). I'm not convinced this issue is not also related to the "phantom" substitution thread in which players are listed in pbp or in the boxscores, but absent from the gamelog.
