You'd have to edit the "nations.txt" file to include the states as "nations", I believe.
As for promotion and relegation, you'll have to make sure you make two separate leagues. At the beginning of pre-season, you can go in and change your league structures and move teams around quite easily.

Backup your cities.txt and nations.txt files, which are in your OOTP root\data\database dir, unzip that download to your OOTP directory (so it overwrites those two textfiles). Restart OOTP and create a new league. You should see all the states on the nations list. Pick Mass and you're all set. You may also want to adjust your foreigner percentage in the league setup, depending on how stringent you are about the odd non-Mass player getting created.
