Few Important Things

1. Custom Playoffs when split-season is in effect

2. Interleague Playoffs

3. Season not every year

4. Multiple Affiliations (Multiple leagues per minor league and multiple teams per minor league team)

5. Sub-leagues with uneven number of teams

#1 is needed for the Mexican League
#2 is needed for the Asia Series and the Caribbean Series ((Asia Series = Australian Baseball League (Winter, MLB-affiliated), Chinese Professional Baseball League, Korea Professional Baseball, Nippon Professional Baseball) (Caribbean Series = Dominican Professional Baseball League (Winter, MLB-affiliated), Mexican Pacific League (Winter, MLB-affiliated), Puerto Rico Baseball League (Winter, MLB-affiliated), Venezuelan Professional Baseball League (Winter, MLB-affiliated)))
#3 is needed for the World Baseball Classic
#4 is needed for Liga Paralela, Dominican Summer League (some seasons), and Arizona Fall League (each team has 6 MLB affiliates)
#5 is needed for the Puerto Rico Baseball League and multiple others

You can do this now; you just have to do it indirectly, and you need a custom-made schedule file (as the game cannot generate schedules for leagues with an odd number of teams).

Start you league with an even number of clubs. Generate the schedule. Delete one club (or shift it to the other subleague, as is appropriate for the setup). Delete the schedule. Import the custom-made schedule for the odd number setup. Play.

How do I make a custom schedule?

That's a complicated question. The file format itself is fairly straightforward. The art of making the schedule is another matter entirely. Folks have different approaches. It's rather like doing a giant jigsaw puzzle.

You may want to stop by the OOTP 11 Mods forum and visit the Schedules section. In that you'll find a ton of custom-made schedule files for OOTP 11 that will work in 12. With any luck you'll find an existing schedule that suits your purposes, or one that it close enough that you can edit it yourself to get exactly what you want.

Promotion and Relegation would be useful as well.

Right? I doubt it will ever be added, though. I'm running a fictional baseball league in OOTP 12 that is based on the English Premier League structure. I'd like to have automatic promotion and relegation as opposed to having to manually move teams around in the offseason. But, we're probably the only ones that would care about such a feature and thus I wouldn't count on it being added.

Moreover, I'd like to see a legitimate international baseball feature added (like the World Baseball Classic, as you mentioned). It would be even more awesome to have international club tournaments in the style of the UEFA Champions League (in addition to their regular leagues, of course).

Again, I doubt it will ever happen, but a boy can dream.

Ya, promotion and relegation is still used in Honkbal Hoofdklasse and most amateur Euro leagues (only Honkbal Hoofdklasse and Italian Baseball League are professional). Italian Baseball League no longer has promotion and relegation to IBL 2D, butt HH does it to the Transition League (while Rookie League serves as an American-style minor league).

You see, add promotion/regulation probably is not really hard, but for that to be meaningful, the team about to be promote/regulate should behave differently than other teams, which is hard to do
